 The Tokaimura criticality accident in 1999 resulted in the deaths of two workers, marking it as a significant incident in Japan's nuclear history. The accident was caused by a criticality event in a fuel preparation plant, where workers mishandled uranium enriched to a high level, leading to a limited uncontrolled nuclear chain reaction. 

A BLUE FLASH seen

The accident highlighted serious safety breaches and human error, with the International Atomic Energy Agency attributing the cause to "human error and serious breaches of safety principles." The incident resulted in a total of 119 people receiving radiation doses over 1 mSv, with two of the doses proving fatal.
